Thank you, It’s been my privilege to serve you and
the medical society this year. The tradition in medical
societies is for the the speech of the outgoing
President to be brief. So I will try to keep mine brief
as well.

Two years ago, Dr. Rich Macchiorelli called me to
ask me to consider stepping into the queue to become
our medical society’s President. My initial response
was unexpected to him – I declined. I explained to
Rich my concern: How can I really represent our
members during this overhaul of the healthcare
system when my practice of cosmetic surgery will be
largely unaffected? If I am not really in the trenches
with our members and suffering with them by having
my bottom line reduced, or being forced to see higher
patient volumes, or being burdened with larger
administrative challenges as they are, How can I
really represent them? How will I be received?

He said that he understood my point; but, he told
me he did not feel the members would view my
selection in that light. I thought about it a bit, and I
reminded myself that our medical society is really about one thing – Advocacy. And advocates of
medicine come in all forms: hospitalists, solo
practitioners, group practitioners, Lee Memorial
Hospital, concierge medicine doctors, fertility doctors,
ER doctors, and even plastic surgeons. I called Rich
back and accepted the position.

In the recent past, we’ve had a fair number of
doctors holding leadership positions in the society
who, like me, are only indirectly affected by the
changes in the healthcare landscape. I have often
wondered why more docs who are being directly and acutely and adversely affected are
not taking these leadership positions in our society. I
can only conclude that, unfortunately, they are too
exhausted trying to keep their practices afloat.
I can’t help but wonder if this was part of the
government’s and insurance companies’ strategies :
Let’s lower the doctors’ reimbursements. They’ll be
forced to take on higher patient volumes and
administrative costs. They simply won’t have the time
or energy to organize themselves. Then, any changes
we want implemented will be met with little
resistance. Whether this effect was planned or not, it
appears to be happening locally.

I realize that, in some ways, I am preaching to the
choir. Those of you here tonight are here because
you are also advocates and you are active in the
society. Tonight, I am asking you to take it one step
further – I urge you to take on these leadership
roles especially if your practice is directly affected by
these changes in healthcare laws. Our incoming
President, Dr. Mouracade, a nephrologist, is one of
those doctors and we look forward to her talk tonight.
This past year we have seen some promising
movement with SGR repeal. Back in May, our medical
society received an email from the bipartisan Senate
Finance Committee asking for our recommendations
regarding physician payments.

Our Board of Governors alongside key members of
our society brainstormed and delivered a response to
the Senators’ questions. Evidently, our sentiments
were echoed by other medical societies as a few
months later, the Ways and Means Committee
approved the proposal to repeal the SGR formula by
unanimous vote and the Finance Committe approved
it by voice vote. Before January 1st President Obama
signed into law the Pathway for SGR reform Act of
2013 which prevented the scheduled 24% Medicare
payment cuts to physicians. More work needs to be
done but these are positive signs. This year we also
watched the Florida Healthcare Reform Bill passed
into law.

Despite these victories, we have battles ahead. We
need to keep fighting. We cannot retreat into our
laboratories, our offices, or our operating rooms. We
cannot say that we are too busy or too tired. We are
the ones who slept in hospitals for 36 hour (or in my
case sometimes 52 hour) shifts. Why would we say
we are tired now?
More work needs to be done. The Florida
Healthcare Reform Bill which I just mentioned was
recently tossed by a Federal Judge on the grounds
that it violated patient privacy laws. This decision is
currently being appealed. ICD-10 is still coming.
How many more hours will this take away from oneon-one
patient care? How much more cost will this be
to the physician? How will any patient benefit from
the addition of thousands of government codes?
This year, we also watched the United Advantage
Program drop many of our physicians. And we have
watched our Medical Society’s Political Action
Committee be required to dissolve along with other
county medical societies’ Political Action Committees
not for financial reasons but because state legislature
was passed ordering us to do so. At times, our trying
to fight against these attacks against our profession
seems like trying to shoot spitballs at godzilla. We
might not be able to kill godzilla, but we can certainly
prevent him from completely destroying our city.

We cannot say that the intent of the changes in
healthcare laws are bad. Although costly, EMR
(electronic medical record) does offer an easy way to
obtain patient records from other practices and helps
organize patient records. The objective of trying to
provide basic health care to all Americans is an
admirable goal. However, physicians need to be
driving the bus on these changes. Our patients trust
us and not the government. We need to lead the
discussions and say no only when no is indicated.
We cannot place the destiny of healthcare in the
hands of those who do not have licenses to practice
medicine nor the education to deliver medical care.
These points bring us back to leadership. Youfarahmand
might wonder how we got involved with the medical
society’s Board of Governors. Basically, we were
tapped on the shoulders by our peers just as Dr.
Macchiorelli tapped me on the shoulder when he
called me, or as I tapped Dr. Mouracade on the
